Blackpool Council is seeking an experienced Provider to deliver a rehabilitation service at Hornby Road in Blackpool; a supported living setting with communal facilities, accommodating up to 5 people with mental health support needs and other complex issues. The Provider will be responsible for supporting individuals through a rehabilitation model of support, with the aim of them moving on to long term living arrangements.
The Provider will have demonstrable experience and expertise in delivering person centred support to Service users with a wide range of Mental Health needs and other complex issues. This will include experience of delivery of similar schemes with a focus on Mental Health rehabilitation and supporting Service Users to safely and sustainably move on to longer term situations.
