- The procurement is to establish a home care framework. The contract will cover the delivery of home care services including reablement and live-in care. The framework will not utilise lots. In line with the Social Care Futures vision of Adult Social Care: 'We all want to live in the place we call home, with the people and things we love, in communities where we look out for each other, doing the things that matter to us.' 'Supporting the life that matters to you'. This vision will be central to the contract as it aims to enable people to continue to live within their own homes for as long as possible, and with as much independence as possible. The contract will also support safe and timely hospital discharge, and therefore contributes to the wider health and social care system.
