The authority is issuing this invitation to tender (`ITT`) in connection with the procurement described in the advertisement placed in the Official Journal of the European Union with reference number 2019/S 135-332421.
The service will aim to assist with the timely discharge of patients from acute hospitals to residential care homes.
The trusted assessor will seek to :
— reduce time from patient becoming medically fit for discharge to arriving at care homes; (Links to high impact change model for — managing transfers of care HICHM 1),
— improve communication between hospital and care homes to aid timely discharge,
— assist care home managers with assessment process (Links to HICM 3 and 6 multi disciplinary, multi agency discharge teams and trusted assessors),
— provide liaison where there is a potential change of needs,
— reduce the need for care home manager to visit hospital,
— if possible leverage working together on this project to encourage wider working together between independent sector providers.
Selection criteria as stated in the procurement documents.
This pseudo framework procurement will divided into 2 stages:
Stage 1: (quality submission) based on general service specification;
Stage 2: (price submission) based on detailed service specification.
The authority is seeking to achieve multi-supplier pseudo framework for use by the authority and other authorities.
The submission process will be opened throughout the length of this agreement and reviewed by an evaluation panel during the assigned time period. This means that this pseudo framework will be opened once a year for new providers to apply and join.
